College Scholarships
At Citizens Memorial Health Care Foundation, we believe in fostering the educational aspirations of our local students. We want to empower and support the academic journeys of talented individuals within our community. The Foundation currently offers two scholarships:
- Medical Excellence Scholarship for students from the CMH service area who are interested in working in the medical field.
- Club 1982 Employee, Children and Grandchildren Scholarship to assist students entering any field or trade.
Medical Excellence Scholarship
The CMH Foundation established the Medical Excellence Scholarship in 1990 to help individuals pursuing degrees or certifications in medical-related careers. The Annual CMH Foundation Medical Excellence Golf Classic raises money for the scholarship.
Application Deadline
Submit completed CMH Medical Excellence Scholarship applications to CMH Human Resources by 5 p.m. on the last day of February. Late applications will not be accepted. Scholarship winners will be selected by late April.

Requirements
Applicants must:
- Be interested in working in the medical field
- Be from the CMH service area
- Be able to return to the service area as a CMH employee after completing their education level
- Have a minimum of a 2.5 non-weighted GPA on a 4.0 scale for high school or post-secondary educational coursework
Qualifying medical-related career programs include:
- Certified Occupational Therapist Assistant
- Certified Respiratory Therapist
- Clinical Social Worker
- Licensed Practical Nurse
- Medical Technologist
- Medical Laboratory Technician
- Nurse Anesthetist
- Registered Nurse
- Occupational Therapist
- Pharmacist
- Physical Therapist
- Physical Therapist Assistant
- Physician
- Radiologic Technologist
- Registered Respiratory Therapist
- Speech Therapist
- Other licensed and/or registered profession as approved
Application Process
A completed application packet includes:
- The completed application
- Three professional references in sealed envelopes
- The applicant's letter of intent
- A copy of the applicant's most recent transcripts (does not have to be official)
A board will review and select applicants to interview and ask for more information. The board will select about 15-20 new applicants every school year.
Scholarship recipients must show acceptance into a program or university before any financial assistance is given.
Club 1982 Employee, Children and Grandchildren Scholarship
The Club 1982 Employee, Children and Grandchildren Scholarship assists students who strongly desire to further their education/trade. Our employees are the backbone of providing compassionate care at CMH. We want to invest back into them and their families for their hard work and dedication to our organization. Donations to the Club 1982 employee giving program fund the scholarship.
Application Deadline
Completed applications are due by 5 p.m. on Feb. 28.

Requirements
Applicants must:
- Must be an employee or a child or grandchild of a Citizens Memorial employee. The employee must be in good standing for a year and have worked 500 hours in the previous year.
- Have a GPA of 2.5 or higher.
- Be graduating high school or currently enrolled in college or a post-high school trade program.
- Not be receiving another CMH scholarship.
- Complete the entire scholarship form.
