Chantel Carter recognized as 11-time CMH DAISY Award nominee
Citizens Memorial Hospital has awarded Chantel Carter, RN, the DAISY Platinum Award after she received 11 nominations for the DAISY Award for Extraordinary Nurses. Chantel is a registered nurse with CMH Hospice.
Chantel has received numerous nominations recognizing her compassion, professionalism and dedication to her patients and their families. She is described as gentle, kind and honest, always taking time to explain each step of care and providing comfort during difficult moments. Families praised her for going above and beyond, calling her “a gift” and “a true and brilliant diamond” for the compassion and understanding she shows.
Whether staying by a patient’s side until the end, praying with families or ensuring they have everything they need, Chantel’s care consistently reflects love and excellence. Many said she felt like family and that her visits brought reassurance and peace.
The DAISY Award honors nurses who consistently demonstrate exceptional skill, compassion and commitment to patient care.

The DAISY Platinum Award is part of the international DAISY Award program that celebrates extraordinary nurses. The Platinum Award honors nurses who have received 10 or more DAISY Award nominations, recognizing their consistent excellence and lasting impact on patients and families.
CMH began presenting DAISY Awards in January 2019 and added the DAISY Platinum Award in May 2025 to recognize this extraordinary level of dedication.
