New Employee Orientation

Congratulations on being selected to join CMH! Please review the information on this page to help make your transition to your new position as smooth as possible.

Before Orientation Day

Human Resources will give you a call with the official offer, schedule pre-employment labs and set up a time to fill out paperwork in their office. Online paperwork will be sent to your email to fill out ahead of time, as well.

These include:

  • Information sheet
  • W-4s
  • I-9 Employment verification
  • Background Authorization
  • Consent form for your contingent offer, drug screen and Family Care Safety Registry
  • Vaccination consent form
  • DMV authorization
  • Waiver of benefits (if working in a PRN status)

You will receive these files from your recruiter listed below.

What to Expect Before Employee Orientation

Pre-Employment Lab Testing. Please bring your photo ID!
  • Blood testing for titers (level of antibodies in your blood). 
  • Urine drug screen.
What to bring to your appointment with Human Resources.
  • A pretty smile! We will take your picture for your employee badge.
  • A voided check/bank deposit information to attach to a form you will complete in the office. 
  • Your driver's license or photo ID.
  • Your Social Security card, birth certificate or U.S. Passport.
  • Proof of your highest educational degree, if applicable.
  • Proof of any other certifications/licensures, including CPR, if applicable.
  • Proof of vaccinations, including COVID, if applicable. 
Long-term care employees: You will also be required to get a TB test.
  • Contact the Director of Nursing at the facility where you will be working to schedule your TB test on the Friday before you start. These results will be read at orientation on Monday.

Orientation Day

New Employee Orientation is held on Mondays from 8 a.m.-5 p.m. in the CMH Broadway Professional Building at 2001 W. Broadway St., Bolivar, MO 65613. Please park in a guest parking space or in one of the open spaces on the east side of the building closest to the gas station.

What you will learn at orientation

We want to make sure you are best prepared for your new role, so you will meet with or learn about the following information throughout the day.

  • Top leadership gives introductions and provides an overview of CMH.
  • CEO Michael Calhoun introduces himself and answers questions.
  • Information services staff guide you in logging into your email and accessing the intranet.
  • Marketing reviews marketing materials and communication and takes a group photo.
  • Safety reviews safety procedures and best practices.
  • Our Benefits Specialist breaks down options for employee programs and benefits offered to you as a CMH employee.
  • The Wellness Program specialist reviews how to better your health and lower your insurance.
  • A Human Resources representative reviews policies and procedures.
  • Employee Health reviews your labs, conducts hand-washing training (if necessary) and goes over/administers vaccinations (if applicable).
  • Education reviews basic training.
  • You will complete required online training modules, known as Healthstreams, that will introduce you to our organization and instruct you on best practices to follow while employed with CMH.

What else you need to know

Other items you will receive:
  • A few CMH goodies to take with you on the job!
  • Parking pass.
  • Login information.
  • Scrub vouchers for qualifying positions.
  • Gas cards for qualifying positions.
What to bring with you:
  • Appropriate clothing (scrubs or business casual).
  • Jacket, if desired.
  • Any documents you may have missed during your paperwork meeting with HR.

After Orientation Day

Make contact with your manager! Human Resources will give you their contact information during your paperwork meeting. Ask about your schedule and where you are to report.

Long-Term Care Employees: You will go to facility orientation on the designated days, or you will attend CNA class if you are pursuing your certification.

Nursing: You will attend nursing orientation for your area of work on the designated day.

Communication you can expect as a CMH employee:
  • Daily organizational updates sent through the Daily Safety Huddle to all employees via email.
  • News and updates sent through the Rapid Pulse via email.
  • Auxiliary updates sent via email.
  • Information regarding community events throughout our service area via email.
