CMH welcomes students for clinical rotations and internships. Before you can begin your rotation or internship, you must submit an application and the required documents. This page gives more information about the requirements and how to submit them online.

If you are a medical student currently enrolled in an M.D. or D.O. program, please follow the instructions on the Medical Student Elective Rotations page.

You will need to complete some paperwork before you begin your rotation at CMH.

Student Application

1. Complete and print the fillable PDF application.
2. Take it to your supervisor to sign the second page.
3. Upload the completed application and required documents using the form below. You also can email the completed application to nicole.breedlove@citizensmemorial.com.

Submitted Documents

Before you can begin your rotation, you must upload copies of these documents using the upload form on this page.

1. Background check. Submit a copy of a recent background check that is less than 1 year old.
2. Vaccination records. Submit a copy of all your vaccinations.
3. Flu vaccination. Submit proof of a current flu vaccination. If you are allergic to the flu vaccine, you must provide a doctor's note for approval.
4. Two-step TB skin test. Submit proof of a negative test that is less than 1 year old.
5. Liability insurance. Submit documentation of liability insurance.
6. Photo ID. Submit a photo for your CMH ID badge. Or you can have a photo taken by Human Resources.

Influenza Vaccination Policy: CMH requires all employees to be vaccinated for influenza annually. Each year, for rotations between Oct. 1 and March 31, all students must show proof of influenza vaccination. Certain documented medical exemptions are allowed.

COVID-19 Vaccination Policy: CMH requires all employees to have proof of COVID-19 vaccination since 8/22/24 or submit a signed declination.
